• Child's Full Name

• Package Selection (Print or Digital)

Package Options:

Print Package: 1 - 8” x 10” Print1 - 5” x 7”

PrintDigital Package: You will receive a high-resolution digital copy of your photo, which you may print and reproduce as often as you like. No physical prints are included.
